Overview

The Account Summary - Account Details screen provides detailed information about an account to which you are authorized. The details that display differ based on the account selected - Fidelity Brokerage/Trust Account, a Fidelity Annuity Account, a Fidelity Insurance Account, a CGF Donor Advised Fund Account, or an Fidelity 403(b)/401(k) Account.

Account Restrictions

NOTE: Please see Account Restriction Codes for the list of possible restrictions.

The Account Restrictions section of the screen displays any restrictions placed on the account by Fidelity. The restrictions are listed as a restriction code along with an associated description, for example: h2: Abandoned Account, Q1: Account Closed, C2: Margin Denied, and A8: Lacking Legal Address.

Account Registration

Field Definition
Establishment Date

Indicates the date on which this account was set up and approved.

Registration Type

Indicates the registration type of the account, such as Corporate, Individual, or IRA Rollover.

Corporate Tax Status Indicates a corporation's tax status; options are: C-Corporation, S-Corporation, and O-Other Taxable (Applicable to legacy LLC accounts taxed as Partnerships). NOTE: This field only displays for accounts with a Corporate or LLC Registration Type.
Account Type

Indicates the account type.

Short Name

Indicates the short name used as an abbreviation for the full registration name. You can use this name to facilitate account sorting and identification.

Last Maintenance

Indicates the last date on which a change was entered to any of the account's information.

Confirm Copies

Indicates the number of copies of trade confirmations to send to the primary account holder, up to 20.

Core Fund

Indicates the core fund security.

Account Agreements

The Account Agreements section of the screen displays the status of applicable account documents, and tax certification.


Field Definition
Application

Indicates the status of the account application at Fidelity, such as "Agreement in Transit" or "On File."

Margin Agreement

Indicates the status of the margin agreement paperwork if the account has requested margin borrowing.

LOI Agreement

Indicates whether a Letter of Intent (LOI) is on file at Fidelity:

Option Status

Indicates the status of the option agreement if the account has requested options trading.

Option Level

Displays the numerical value that represents the level of options trading if the account is approved to trade options.

  • 1: Covered writing of equity calls.
  • 2: Level 1 plus purchase of equity and index calls and puts, and purchase of equity and index straddles or combinations.
  • * 3: Level 2 plus equity spreads and covered writing of equity puts.
  • * 4: Level 3 plus uncovered writing of equity calls and puts, and uncovered writing of equity straddles or combinations.
  • * 5: Level 4 plus uncovered writing of index calls and puts, index spreads, and uncovered writing of index straddles or combinations.

* Requires a margin agreement be on file with Fidelity.

Approved Option Trades

Describes any further trading capability beyond the general Option Level.

Commission Discount

Indicates the security types in which the account's trades are eligible for a commission discount.

Discount Percentage

If eligible for a commission discount, displays the percent of that discount. This value cannot exceed 100%.

Schedule Code

Displays the two-digit code that identifies the commission table used to calculate commissions for the account.

International Trading

Indicates whether or not the account is enabled for international trading.

Securities Lending Agreement

Indicates whether or not a signed Securities Lending Agreement is on file for the account.

Cost Basis Disposal Method

Indicates the cost basis disposal method on file for the account; First-In, First-Out (FIFO) is the default. NOTE: See Cost Basis Disposal Methods for detailed descriptions of the available disposal methods.

Omnibus Indicator

NOTE: This information displays for Administrator, Bank, Corporate, Fiduciary, Limited Liability Corporation, Non-Prototype, Non-Prototype IRA, Professional Association, Professional Corporation, and Trust Under Agreement Registration Types.

Omnibus accounts, otherwise known as pooled accounts, are defined as a single account that pools the holdings of more than one beneficial owner, whose identity is not disclosed to Fidelity, who has the ability to affect transactions, and for which sub-accounting is performed by the omnibus account holder or a third-party. This may include, but is not limited to, retirement plans, bank pooled accounts, and wrap programs.

An Omnibus Account Certification Form must be on file with Fidelity for your firm to code accounts with the values 01, 02, and 03.

Omnibus Status

Displays a code indicating if the account is an omnibus account:

  • 01: Not an omnibus account - only one underlying shareholder
  • 02: Not an omnibus account
  • 03: An omnibus account and intermediary can and will track, collect, and remit short-term redemption fees at the sub-account level
  • 04: An omnibus account and intermediary cannot and will not track, collect, and remit short-term redemption fees at the sub-account level. Accounts with this code are blocked from future purchases of Fidelity funds with short-term redemption fees.
  • 05: Pending, status unknown

Payout Instructions

Field Definition
Sale Proceeds

Indicates what to do with the cash proceeds of a security sale:

  • Remit Proceeds: Proceeds less than $25 remain in the account. Proceeds between $25 - $10,000 are automatically sent via a check to the name and address of record. Proceeds greater than $10,000 are automatically remitted to customer if a result of trade-related activity.
  • Hold Proceeds:
    Proceeds held in the designated interest-bearing core money market. Retirement accounts must have instructions set to Hold Proceeds.
  • DVP/RVP:
    Proceeds delivered according to delivery instructions. This instruction choice uses the DTCC-ID system to settle sell trades electronically.
Securities

Indicates what to do with the physical securities when they are purchased in the account:

  • Transfer and Ship:
    Certificates are registered in the name of the account holder and sent to the address of record.
  • DVP/RVP:
    Uses the DTCC-ID system to settle buy trades electronically.
  • Hold in Street Name:
    Certificates are held in custody for the investor.
Dividend Instructions

Indicates how to distribute the account's dividend income:

  • Pay Weekly:
    Dividends post to the dividend disbursement account (Type 9) and are paid weekly on Friday.
  • Pay Semi-Monthly:
    Dividends post to the dividend disbursement account (Type 9) and are paid the second Friday and the last Friday of each month.
  • Pay Monthly:
    Dividends post to the dividend disbursement account (Type 9) and are paid the last Friday of each month.
  • Pay Quarterly:
    Dividends post to the dividend disbursement account (Type 9) and are paid quarterly on the last Friday of the following months: March, June, September, December.
  • Hold in Account:
    Dividends post to the cash account (Type 1) and held in the designated core money market or as a credit balance if no core account exists.
  • Reinvest Eligible Equity & Mutual Fund Dividends:
    Dividends processed according to the sales proceeds instruction. Eligible mutual fund and equity dividends reinvested.
  • Reinvest Eligible MF Dividends:
    Dividends processed according to the sales proceeds instruction. Eligible mutual fund dividends reinvested.
  • Reinvest Eligible Equity Dividends:
    Dividends processed according to the sales proceeds instruction. Eligible equity dividends reinvested.

Field Definition
Trading Level

Displays the name of the authorized individual and the associated level of trading authority, such as Full, Limited, Discretionary, or Power of Attorney.

Program Name

Displays the nine-character ID that identifies the managed account program name.

Separate Account Manager

Displays the full name of the professional account manager for the managed account program.

Investment Discipline Describes the manager's defined approach to managing account assets in the program.

Fidelity Annuity Account Details

You can view detailed information about a selected deferred or income annuity contract. The Product Name, which is the legal name of the insurance product, and Product Type display at the top of the screen.

Account Owner

This section of the screen displays the full name and address, including city, state, and zip code, for the primary account holder.

Annuity Income Information

This section provides additional information if the annuity contract is in the payout phase or if the product type is a fixed or variable income annuity.


Field Definition
Income Value

Indicates the current value of the next income payment.

Guaranteed Period (years)

Indicates the minimum period of time (in years) that income is guaranteed to the annuitant and their heirs.

First Pay Date Indicates the date on which the first income payment was calculated and generated.
Insurance Company Assets

Indicates the assets which the insurance company has set aside to provide income for the annuitant's lifetime or the guaranteed period, whichever is longer. The Insurance Company Assets are not available for withdrawal from the contract. The only assets available for withdrawal from the contract, if any, would be displayed in the Liquidity Value field.

Withdrawal Option

Indicates whether or not the contract owner has chosen the ability to make withdrawals from the variable investment options of the income annuity.

Values are Yes and No.

Next Pay Date

Indicates the date on which the next income payment will be calculated and generated.

Liquidity Value ($) Indicates the withdrawal value, if any, which is the total amount that can be taken out of the income annuity during the withdrawal period.
Income Options

Indicates the type of annuity income option such as Single Life or Joint and Survivor that was chosen by the contract owner.

Frequency

Indicates the payment frequency chosen by the contract owner.

  • Monthly
  • Quarterly
  • Semi-Annually
  • Annually
Benchmark (%)

Indicates the rate of return that was chosen by the contract owner to be used for calculating the initial annuity payment and future income payments. This represents the earned annual total returns needed (less expenses) to keep income payments level.

Registration Indicates the type of ownership registered for the account, such as Individual, Joint or Trust.
Source of Funds

Indicates whether or not the annuity contract was funded with qualified or non-qualified assets.

Contract Date Indicates the date that the contract was issued.

Indicates whether or not the contract owner has chosen to make automatic monthly exchanges from either the Money Market investment option or the Investment Grade Bond investment option to other selected investment options.

Automatic Rebalancing

Indicates whether or not the contract owner has chosen to have their contract holdings allocated on a scheduled basis to return to selected allocations.

Values are Yes and No.

Systematic Withdrawal Plan

Indicates whether or not the contract owner has chosen to have a specific dollar amount automatically withdrawn from the contract on a selected periodic basis.

Electronic Funds Transfer

Indicates whether or not the contract owner has chosen to make regular deposits to the annuity contract by pre-authorized transfers from a bank account.

Field Definition
Owner I

Indicates the name, date of birth (DOB) and social security number (SSN) of the primary contract owner with whom the insurance company makes the contract.

Owner 2

Indicates the name and date of birth of the joint contract owner.

Annuitant 1 Indicates the name and date of birth of the primary person designated by the contract owner, upon whose life annuity payments are based.
Annuitant 2

Indicates the name and date of birth of the joint annuitant designated by the contract owner, upon whose life annuity payments are also based.

Account Owner

This section of the screen displays the role and the full name and address, including city, state, and zip code, for the primary account holder.

Field Definition
Policy Date Displays the date that the insurance coverage became effective.
Face Amount Displays the current dollar amount of insurance on the policy.
Death Benefit

Indicates whether or not the contract owner has chosen the ability to make withdrawals from the variable investment options of the income annuity. Options are:

  • GMDB
  • Level
  • Optional
  • Standard
  • Variable
Death Benefit Option Displays the death benefit options such as level benefit or variable benefit that was chosen by the policy owner.
Loan Balance Displays the principal amount plus accrued interest due under all loans currently outstanding against this policy.
Cash Surrender Value Displays the total value of the current balances in the investment options, the fixed account, and the loan collateral account less any loan balance and charges due.
State Premium Tax Rate (%) Displays the tax rate assessed by state and local governments against life insurance premiums paid.
Scheduled Premium

Displays the premium amount for which the policy owner is currently being billed.

Premium Mode

Displays the frequency of premium payment that the policy owner is being billed. Options are:

  • Monthly
  • Quarterly
  • Semi-Annually
  • Annually
Source of Monthly Deductions

Displays the fund or, if pro-rata, funds from which the monthly deductions of policy charges are taken. Options are:

  • Money Market Fund
  • Pro-rata

Modified Endowment Contract (MEC) Status

Displays whether or not the policy qualifies as a modified endowment contract, as defined under the Internal Revenue Code. Values are Yes and No.

Dollar Cost Averaging

Indicates whether or not the policy owner has chosen to make automatic monthly exchanges from either the Money Market investment option or the Investment Grade Bond investment option to other allowed investment options. Values are Yes and No.

Automatic Rebalancing

Indicates whether or not the policy owner has chosen to have their contract holdings allocated on a scheduled basis to return to selected allocations. Values are Yes and No.

Disability Waiver Monthly Deductions Rider

Indicates whether or not the policy owner has chosen the rider which provides that monthly deductions under the policy will be waived while total disability continues and if other requirements are met. Values are Yes and No.

Disability Premium Payment Rider Insured 1

Indicates whether or not the policy owner has chosen the rider which provides that the insurance company will pay a disability benefit amount to the individual indicated as Insured 1. Values are Yes and No.

Disability Premium Payment Rider Insured 2

Indicates whether or not the policy owner has chosen the rider which provides that the insurance company will pay a disability benefit amount to the individual indicated as Insured 2. Values are Yes and No.

Children's Term Rider

Indicates whether or not the policy owner has chosen the rider which provides term life insurance coverage on each of the insured's children. Values are Yes and No.

All assets held in a Giving Account® are property of the Trustees of the Fidelity Investments Charitable Gift Fund. The balances do not represent assets of the Donor. Gift Fund Donors have advisory privileges only over funds held in a Giving Account.
